Invitro Fertilisation(IVF) / Intracytoplasmic Sperm Injection(ICSI)

IVF is a process where an egg is combined with sperm outside the body. IVF occurs via natural penetration of sperm in to egg. In IVF, there should be at least 50,000 motile sperms per egg.

ICSI is a specialised form of IVF in which embryologist selects morphologically normal sperm and inject it directly in to the egg under micromanipulator, a specialised microscope with high magnification.

Once fertilised by IVF or ICSI, the resultant embryos are cultured till day 5 before being transferred into the uterus.

Indications For IVF:

  • Younger women with early ovarian aging
  • Older women with low ovarian reserve
  • Blocked or damaged fallopian tubes
  • Endometriosis
  • Oligoathenoteratozoospermia ( sperm count <8mill/ml, >60 to 70% immotile sperms, 1% normal morphology)
  • Unexplained infertility
  • As a part of fertility preservation for patients who are undergoing chemo or radiotherapy
  • As a part of social freezing

Indications For ICSI

  • Severe OAT
  • Azoospermia sperm completely absent from ejaculate in which Sperm retrived from surgical technique
  • History of fertilisation failure in conventional IVF attempts
  • If PGD is being used to screen embryos for specific genetic disorder
  • Globozoospermia in which sperm is morphologically abnormal ( lack of acrosome)

Steps involved in IVF

  • Couples detailed history, hormonal and metabolic profile are evaluated.
  • 3D baseline ultrasound is done for the female partner to know AFC and rule out any uterine structural abnormalities.

Hormonal injections are administered daily for 10 to 12 days depending on the individual patient response to produce multiple eggs.

Regular monitoring through blood tests and ultrasound is done to know the rise in hormones and growth of follicles.

once the follicles reach an appropriate size and estradiol levels are adequate, we will administer a trigger shot which induces final maturation of egg with in follicle.

It is a minor surgical procedure performed under sedation to retrieve eggs 34 to 36 hours after a trigger shot under ultrasound guidance.

  • IVF: Mature eggs and processed sperms are combined in a lab
  • ICSI: A single morphologically normal sperm is injected into each mature egg

Embryos are cultured till day 5 and the resultant embryos are graded by embryologist.

Embryos will be transferred back to the mother’s womb either in the same cycle (FRESH EMBRYO TRANSFER) or later (FROZEN EMBRYO TRANSFER) under ultrasound guidance.

Hormonal support is given to aid implantation.

Beta HCG test is done 14days postembryo transfer to confirm pregnancy. Follow up ultrasounds are done to monitor the progress
